Admission criteria

I
n order to be eligible for Clubhouse membership, individuals must meet the following minimum criteria. They must:
1. Have experienced a brain injury and be at least 18 years of age.
2.
Be free of severe behavioral challenges that would disrupt group activities or cause a threat to others.
3. Be willing to commit to the Clubhouse Substance Abuse Policy.
4. Arrange his/her own transportation to and from the Clubhouse.
5. Have no medical problems that interfere with participation in the Clubhouse.
6. Be capable of meeting personal needs and basic self-care independently.
7. Be able to communicate with others through speech, assistive devices.
8. Be willing and able to follow Clubhouse guidelines.

Admission/referral process

A
referral can be made by an individual living with the effects of an acquired brain injury, family member, or service provider.
1. Call Cornerstone Clubhouse (679-6809) for more information about the program and set up a tour.
2.
If you decide you are interested in becoming a member, request an application package.
3. Once the initial application form has been completed, call Cornerstone Clubhouse to arrange an orientation appointment.
